TWO-YEAR FAITH FORMATION REQUIREMENT
Any student who wishes to receive sacraments here at Immaculate Conception Church is expected to be enrolled in his/her 2nd year of an age-appropriate Faith Formation Program to be eligible to receive the Sacrament(s). Please, make sure to register your child for Faith Formation or contact the Faith Formation Office for more information.

BAPTISM INFORMATION
You will need the child's Baptism date and location in order to complete this registration form; in addition:
- If your child was baptized at Immaculate Conception:
  - Provide an accurate date of Baptism.
- If your child was baptized in a church other than Immaculate Conception:
  - If registering for First Reconciliation, please submit a copy of their Baptismal Certificate to the Parish Office by August 22 via email to [email protected] (attached, high-resolution .jpg or .pdf file) or by mail to Immaculate Conception Church, 216 East Dunstable Rd., Nashua, NH, 03062.
- If your child was not baptized in a Catholic Church:
  - Please contact the Faith Formation Office.
HOW TO DETERMINE THE SACRAMENT(S) FOR WHICH TO REGISTER YOUR CHILD

- First Reconciliation (Grade 2 and up)

*** Registration for First Reconciliation is closed.
- If your child is baptized and in Grade 2 and will be enrolled in his/her second year of Faith Formation, he/she is eligible to receive First Reconciliation this school year. This will be the only Sacrament the students will receive during Grade 2. Materials will be provided.
- The deadline to register for First Reconciliation is August 22, 2024.
- Confirmation/First Eucharist Preparation (Grade 3 and up)
- If your child is baptized, has received First Reconciliation, is in Grade 3, and has been enrolled in (at least) his/her 2nd year of Faith Formation, he/she is eligible to receive Confirmation/First Eucharist this school year.
Why is Confirmation received in Grade 3?
In September 2020, we held our First Restored Order Confirmation/First Eucharist Celebration. What does that mean? It means that we are now under a new initiative called “Restore • Renew • Proclaim the Hope that is Christ” from Bishop Libasci in Manchester. Although it has been a transition from what we as a Parish and as Catholics are used to, we have embraced it, and as the Bishop states in his letter "A Call to Hope, A Call to Act A Pastoral Letter from Bishop Peter A. Libasci to the people of the Diocese of Manchester January 16, 2019," there are many blessings being realized through this implementation.
Therefore, from now on, children in Grade 2 will be prepared for the Sacrament of First Reconciliation only, and in Grade 3, the children will be prepared for both the Sacraments of Confirmation and First Eucharist, which will both be received at the same Mass, usually in May of their Grade 3 school year.

- Confirmation Only (if your child has already received First Eucharist)
If your child has already received the Sacraments of Baptism, First Reconciliation, and First Eucharist, but not Confirmation, and is in Grade 8 or below, and has been enrolled in Faith Formation for at least two years (or in his/her second year), he/she is eligible to prepare to receive Confirmation in the Spring of 2025.

High School Students and Young Adults seeking the Sacrament of Confirmation should contact the Parish Office.
Fees and Dates
- Cost:
  - First Reconciliation - $50/Student
  - Confirmation/First Eucharist - $100/Student
  - Confirmation Only- $50/Student
- The Sacrament of First Reconciliation will be held in September 2024.
The Registration deadline for all students who want to receive First Reconciliation is Thursday, August 22, 2024. If registration is not received by this date, your child will need to wait for the next year's preparation and celebrate in the Fall of 2025.

The First Reconciliation Preparation Program dates are Wednesday, September 11 and 18, at 6pm, with the First Reconciliation on Wednesday, September 25, 2024, at 6pm.
- The Sacrament of Confirmation and First Eucharist will be held in the Spring of 2025.
The Registration deadline for all students who want to receive these Sacraments is Thursday, October 2, 2024. If registration is not received by this date, your child will need to wait for the next year's preparation and celebrate in the Spring of 2026.
